  • Detailed communication: Initial evaluation report and progress updates sent to your office on request, faxed or via secure email.
  • Fast access: Most patients seen within a week. Free 15-minute phone consultations available for patients on the fence about starting.
  • Whole-system care: We treat the pelvic floor as part of the larger orthopedic and neuromuscular system. Patients with hip, low back, SI, or coccyx symptoms often benefit from our integrated approach.

How to Refer

Option 1 — Email or fax a referral

Email info@foundations-pt.com or fax (914) 478-0138 with the patient's name, contact info, and brief clinical context. We'll reach out to the patient directly to schedule.

Option 2 — Have the patient self-refer

Patients can book online at rspt.janeapp.com or call 914-768-3802. Most insurance plans in New York allow direct access — no referral required to start.

Option 3 — Three-way conversation

For patients with complex presentations or who would benefit from coordinated care, we're happy to schedule a brief provider-to-provider conversation before the first visit. Just call our office.

What Patients Pay

We are out-of-network with commercial insurance and accept Medicare as a non-participating provider. Most patients with out-of-network benefits receive significant reimbursement. We verify benefits for patients before their first visit and submit out-of-network claims on their behalf.

Communication Back to You

By default, we send an initial evaluation summary to referring providers within 72 hours of the first visit and progress updates at meaningful clinical milestones. We can adjust frequency and format to your preference — just let us know.
