Conditions We Treat
At Foundations Physical Therapy, we treat low back pain, hip pain, and hip labral tears, as well as coccyx (tailbone) pain, Femoral Acetabular Impingement (FAI), and sacroiliac (SI) joint pain. Our therapists are experienced in evaluating and treating sciatica and nerve referral pain, groin and pelvic pain, and chronic musculoskeletal aches that haven’t responded to other care. We also specialize in pelvic floor-related orthopedic diagnoses, post-surgical rehabilitation, and functional movement limitations — including difficulty squatting, lifting, or running.
Movement Without Limits
Persistent pain in your back, hips, or groin can be referred from structures within your pelvic floor or surrounding areas. Nerves that travel through the pelvic floor can be irritated from pelvic floor muscle dysfunction and result in pain in regions of the body sometimes associated with other musculoskeletal injuries such as sciatica or a hamstring strain.
Pain in the following areas of the lower body can be a result of referred pain from the pelvic floor: the lower abdomen, groin, hip, lower back, sacrum, tailbone, or down the back or side of the leg. Sometimes these areas of pain can be confused with gastrointestinal issues or other musculoskeletal injuries but when it is not improving and no one is “touching” the pain it may be coming from an area of the body that is not being evaluated.
When your pain is not responding to traditional treatments it can be confusing and you may feel dismissed by your providers. The reality is the pelvis and surrounding muscles play a large role in common areas of pain, and our patients are often relieved to have found the source of their symptoms.
At Foundations Physical Therapy, we combine orthopedic treatment and evaluation techniques with pelvic floor expertise, we find often overlooked contributors to pain to restore strength, coordination, and function.

Standard Insurance PT
- Time: 15 mins with a PT, 45 mins with an assistant or aide.

- Time: 60 minutes, 1:1 with your specialized Pelvic Floor PT.

Low Back Pain & Hip Dysfunction
Low back and hip pain are common experiences, but traditional care often misses how underlying pelvic floor issues can contribute or cause persistent discomfort. Pelvic floor dysfunction can result in lower back pain and deep hip pain that mimics other musculoskeletal diagnoses.
Our Approach: Through manual therapy, exercise-based rehabilitation, and functional movement retraining, we address muscles, connective tissues, and nervous system coordination to relieve pain, restore strength, and improve mobility.
Sciatica & Referred Pain
Sciatic nerve pain refers to pain due to entrapment or irritation of the sciatic nerve. This can occur because of an injury anywhere along the length of the nerve originating from the spine and traveling all the way to the foot. In some people this nerve can travel through some of the pelvic floor muscles and be irritated by them if they are tight, nonrelaxing and inflexible. Pain from the sciatic nerve can result in pain in the hip, down the back of the leg, into the calf or just show up in the foot. It can lead to weakness and sensory changes if severe and left untreated.
Referred pain generally does not follow a nerve path but a myofascial path that is sometimes predictable and sometimes not. Pelvic floor pain referral patterns are generally into the lower abdomen, lower back, hip, groin, or down the leg. The referral patterns can sometimes make it very confusing to figure out where the pain is originating from and until someone touches the structure doing the referring, you may not be aware of that area of dysfunction.
Our Approach: By taking a multipronged approach to experiences of pain, we identify referral patterns and provide targeted pelvic floor and orthopedic interventions to relieve pressure, improve muscle coordination, and restore movement without fear or limitation.

Post-Surgical & Chronic Pain Rehabilitation
After surgery or periods of persistent pain, individuals often experience weakness, stiffness, or movement restrictions that, without targeted rehabilitation, can become long-term barriers to function and comfort.
Our Approach: Our practitioners guide patients through progressive rehabilitation that combines orthopedic exercise, pelvic floor integration, and movement-based therapy to address stiffness, scar tissue, and neuromuscular imbalances while reducing long-term vulnerability to recurring pain.

Traditional PT may focus primarily on the spine or leg muscles, but pain can refer from the pelvic floor and surrounding core muscles. Yes. Our goal is to restore full functional capacity. Patients regain the ability to squat, lift, run, and participate in daily activities without pain or limitations.
No. While many pelvic floor services focus on women, pelvic floor dysfunction can contribute to pain in anybody. Our clinic treats all adults, integrating pelvic floor and orthopedic care to improve movement, reduce pain, and restore functional strength.
Recovery depends on your surgery type and individual progress, but our rehabilitation plans focus on gradual, safe progression. We rebuild strength, flexibility, and movement patterns so patients can eventually return to exercise, lifting, and daily activities without reinjury. We work with your other providers as well to get guidance on post-surgical timelines of recovery.
Yes. We gladly accept Medicare patients looking for expert, specialized care. We are a non-participating Medicare provider which means we collect payment at time of service (on a Medicare fee schedule), after which Medicare and your secondary insurance will reimburse you directly. This is different from some other providers who will accept only a copay. Because we are not a high volume clinic, participating in Medicare in this way is the only way we can serve this population.
We do not accept insurance outside of Medicare plans. Our comprehensive approach to treatment provides 1:1 pelvic and orthopedic healthcare that standard insurance-based models cannot support. However, we do accept out-of-network benefits and can bill towards your out-of-network insurance on your behalf.
